Complete our intake form and submit an appointment request. You will be asked to provide some basic information on you and the services you require for your pet.
This form goes to our Service Operations team and they will follow up with you to coordinate schedules and arrange the appointment.
You can also call our Pad Service Operations team directly at 347-767-7641 to book your appointment.
Phone Assessment
If you booked your appointment on-line, our Customer Care Team team will follow up with you over the phone to learn more about your pet (e.g. Current medical needs, veterinary instructions, behavioral issues, etc.). We will also confirm your location, provide an estimate for the duration of the service appointment and any specifics that our Vet Technician will need to know prior to the visit.

We will provide some basic pre-appointment instructions to ensure you and your pet are ready for our Veterinary Technician to visit your home.
Appointment
Our Veterinary Technician will arrive at your home during the scheduled appointment time slot. Please ensure that the Appointment Preparation Steps are complete prior to the appointment to ensure you and your pet are all set when our Veterinary Technician arrives.
Visit Summary
& Follow-Up
After the service appointment is complete, we will follow-up with a visit summary, instructions (as appropriate) and a follow-up appointment recommendation (if required).
